Brief
This product is an app designed for completing health screening questions before your appointment date. The app also provides features such as viewing your screening history, doctors, and other appointment information.
Problem
Check-ins and paperwork can be overwhelming, especially on arrival. New and returning patients want to skip the check-in process for their appointments to save some extra time.
Goals
The goal is to design a prototype for an app that will allow users to answer screening questions, track their history, and view their upcoming appointment information.

Features
Main Elements
Users can easily access their screening history, profile page, and start new assessments with the plus icon on the minimally designed nav bar.
Screening Process
As new users go through the screening questions, there is a progress bar guiding them throughout the process. Users may also return to previous pages to make changes if necessary.

All screening questions are saved and users may return to their last spot.
